We handle the specialized education and symptom management so you can focus on your patients' broader healthcare needs. Detailed consult notes are always securely faxed back to your office following assessments to ensure seamless continuity of care.

How to Refer a Patient

Our referral process is designed to be seamless for your administrative staff. All consultations are conducted virtually.

Step 1: Download the Referral Form or Interactive Referral Form

Step 2: Attach Documents: To ensure safe and accurate prescribing, please include the following (if available) with the completed form:

Cumulative Patient Profile (CPP)
Most recent medication list
Any relevant consult notes, lab results, or imaging

Step 3: Fax the Referral Fax the completed form and supporting documents to our secure clinic line:

Once received, our administrative team will contact the patient directly to guide them through our virtual onboarding process and schedule their initial consultation.
